Player A has too many tilesPlayer A’s hand is declared dead, and they stop playing. 
Player B has too few tiles

Player B’s hand is declared dead, and they stop playing. 


Player C and Player D continue playing

 
Player C has invalid exposuresPlayer D declares Player C’s hand dead, and all hands are thrown in.

This also applies to a game with only three players. If two players have problems other than mah jongg in error, all the tiles are thrown in, and the game starts over.
